Noninvasive ventilatory support is rarely used in a general anesthesia environment. However, when it is used, it is usually for urgent situations in which a difficult airway is expected. The objective of this study was to describe the use of noninvasive ventilation during a general anesthesia for airway emergencies in children. Retrospective review of all cases of airway emergency in which noninvasive ventilation was used during general anesthesia for a pediatric population between January 1, 2011, and December 31, 2012. The medical record of every patient in whom noninvasive ventilation was administered for a pediatric airway emergency was reviewed. General anesthesia was used in 377 cases during the study period. Noninvasive ventilation was used in 52 cases, and a combined strategy was used in 9 cases, all of which occurred in neonates (median age 1.1 months, range 0.6-4.3 months; median weight 3.6 kg, range 1.9-8.2 kg). Twenty-three of the 52 patients in the noninvasive ventilation group had airway obstruction, and 28 had cardiac arrest. In the noninvasive ventilation group, the most common etiology was apnea of prematurity, which occurred in 14 of the 23 neonates. The duration of noninvasive ventilation support in the neonate population varied (median duration 4.3 h, range 1.6-6.3 h). In this study, we described the case of a 0.6-month-old neonate who received noninvasive ventilation for airway obstruction due to aspiration of gastric contents. We also reviewed the use of noninvasive ventilation in an attempt to describe a protocol to facilitate its use for airway emergencies during general anesthesia.
